An exciting opportunity has arisen to join the Adult Social Care Team. The post is for an NMC registered Nurse, it is a permanent, full time position. Experience of working with Adults is essential and it would be highly advantageous if you have worked in the community supporting clients with complex health and social care needs. Your key role and function will be to carry out strengths based Social Care Assessments and develop support plans that are person centred, enabling and outcome focused. Capacity and Best interests decision making, as well as the safeguarding of adults at risk of, or experiencing abuse, will all form part of your work with the team. We are a dynamic and supportive team of Social Workers, Nurses, Assistant Social Care practitioners and Support Workers.
