Jersey is on an exciting journey towards implementing a new model of care including the build of a new hospital that will transform health and social care provision in Jersey. The overarching aim of the Jersey Care Model is to transform health and social care, in order to improve Islanders’ physical and mental health and wellbeing. To achieve this, it adopts a person[1]centred approach whereby care is financially sustainable, safe and accessible, being provided in the places where people need it the most. As the Head of Transformation, Commissioning and Partnerships, you will be responsible for leading the implementation of the commissioning strategy and partnership approach across the island in order to deliver the transformational change described within the Jersey Care Model.
